Step 2: Bring all required documentation/forms (
see below) to your scheduled appointment.
Required Forms:
1. Picture Identification for the custodial parent
2. Birth Certificate
- An original birth certificate
- Passport
- An original or attested transcript of a baptism certificate or other religious record showing the date and place of birth of the student
- I-94; Permanent Resident Visa or Green Card
3. Immunization/Medical Records (Click here for forms)
- This information must be presented within 14 days after student’s first day of attendance or student will be excluded from school
4. Divorce/Custody Documentation (if applicable) – must be court signed and date/time-stamped
- Final Divorce Decree which may include Shared Parenting Plan naming specific parent as residential parent for school placement
- Grandparent Power of Attorney
- Temporary Custody Order naming specific guardian
- Military Power of Attorney
5. Verification of Residence* ~ Two (2) Proofs required: (Click for printable list)
- Purchase contract agreement/Settlement statement to buy/lease property (within 60 days of closing); or
- Rental/Lease agreement with custodial parent’s name (and ALL adult residents) listed; or
- Current (within 30 days) public utility bill (water, electric, gas, or cable bill); or
- Government mailing (child support, government assistance, etc); or
- Driver’s license/State ID card with address of new residence
Within thirty (30) days of your change of residency, you will need to complete the Change of Address form and bring it, along with two (2) proofs of residency, to the Enrollment Center or your residency may be in question.
