Are You Moving?  Have You Moved?

The Change of Address Form may be found on the right side of this Enrollment page (under Enrollment Documents).  The completed form, along with required documentation, must be submitted to the Enrollment & Family Resource Center for processing. Failure to update your address within the first 30 days following your move could result in the denial of your open enrollment request and provide a delay in receiving school communications and transportation assignments.


Pre-School Enrollment

You must contact the Preschool Department at 614.797.5903 or 614.797-7452 prior to the completion of the online enrollment application.

Enrolling your child in Westerville City School District

All students new to the Westerville City School District will register at the district’s Enrollment and Family Resource Center, located inside the OhioHealth Westerville Medical Campus. Please call (614) 797-7700 with any questions.

Step 1: Complete the online enrollment application.
Step 2: Once you have completed your online Enrollment Application you will be instructed to schedule your registration appointment with an Enrollment Specialist. (Location & Directions)
Step 3: Bring all required documentation/forms (see below) to your scheduled appointment.

2015-2016 Kindergarten Class

Please do NOT schedule your appointment for enrollment through the online application, but plan to attend Kindergarten 101 and schedule your appointment that night.    If you do not attend Kindergarten 101, you may call the Enrollment Center at 614-797-7700, beginning MARCH 12th, to schedule.

  • Kindergarten is required for children before entering first grade.
  • Kindergarten registration begins March 16, 2015.
  • If your child will be five years old on or before September 30, 2015, you may register him/her for kindergarten in the Westerville City Schools.
    • Those who wish to apply for the All-Day Kindergarten Lottery may do so in March. Additional information will soon be available on this web page.
  • If your child will be five years old after September 30, 2015, but you believe he/she is ready for kindergarten, please contact our Gifted & Talented Services Department at 614-797-5887 to discuss early entrance.
  • If you are unsure of which elementary school your child would attend based upon your home address, please contact any of our elementary schools or the Enrollment & Family Resource Center at 614-797-7700.
  • If your child is a currently-enrolled Westerville City Schools preschool student, you do not need to re-register.
    • Please pick up kindergarten information at any elementary school, the Early Learningn Center, the Enrollment & Family Resource Center, or during Kindergarten 101.
  • Kindergarten 101 will be held at 7 p.m. on March 10, 2015, at both Fouse Elementary School and Hawthorne Elementary School.
    • This event clarifies the process for enrolling students in kindergarten, explains the kindergarten day and student expectations, and offers parents the opportunity to ask questions about the kindergarten experience. Parents also will be able to schedule an appointment for enrollment.


Westerville Schools are excited to begin Online Registration for our 2014-2015 incoming students! Parents will now complete the enrollment information online, instead of the previously utilized paper enrollment packet.

To begin, please click on the Enrollment button below. You will be instructed to create a log-in and password, which allows you to begin the process, save your information, and return at a later date for completion (if necessary). At the time of your enrollment appointment*, please bring ALL required documentation with you to complete the enrollment process for your student.

*If you are 10+ minutes late to your scheduled appointment, please understand we will be unable to honor your appointment time, but will be happy to reschedule for a new date and time.

Registration for the 2015-2016 school year is not yet under way.


Information For Students Entering Westerville City Schools From a Foreign Country 

Students who are entering the Westerville School District from a foreign country must provide documentation of a negative TB test (Mantoux PPD preferred) or a negative chest X-ray from a physician or clinic in the United States prior to starting school. 

The test must be completed before scheduling an appointment for enrollment.  The negative test results must be presented, along with your other required documentation, during your enrollment appointment.

TB skin tests may be obtained at: 

  • Family doctor’s office 
  • The Little Clinic at Kroger’s 
  • Take Care Clinic at Walgreens 
  • Minute Clinic at CVS 
Required Documentation for Enrollment
1. Birth Certificate
  • An original birth certificate
  • Passport (If United States passport, must provide birth certificate)
  • I-94 card; Permanent Resident Visa, or Green Card

2. Immunization Records (Click here for forms

  • Required by the State of Ohio

3. Medical Records - Records of chronic medical conditions (if applicable)

4. Divorce/Custodial Documentation (if applicable) – Must be court signed and date/time stamped

  • Final Divorce Decree which may include Shared Parenting Plan, naming specific parent as residential parent for school placement
  • Grandparent Power of Attorney
  • Temporary Custody Order naming specific guardian
  • Military Power of Attorney

5. Current Individual Education Plan (IEP) / Multi-Factored Evaluation (MFE) / 504 Plan – (If this applies to your child)

6. Photo Identification for Custodial Parent

7. Verification of Residence – Two (2) proofs required

*If your mortgage statement or utility bills are processed through a type of online payment, it is your responsibility to obtain current statements, as required.

Choose your first (1st) proof from the A list; the second (2nd) proof must be provided from the B list:

  1. Options for first (1st) Proof of Residency:
    1. Up-to-Date* Rental/Lease Agreement with custodial parent’s name (and ALL adult residents) listed; or
      *If on month-to-month lease, please bring current verification from landlord, including landlord’s contact information, on business letterhead
    2. Current Mortgage Statement with custodial parent’s name listed; or
    3. Purchase/Contract Agreement to buy/lease property (if within 90 days of enrollment); AND
  2. Options for second (2nd) Proof of Residency:
    1. Official Confirmation of address change filed with the US Postal Service mailed to your new address;
    2. Current (within 30 days) Water Bill, showing service address at Westerville Schools’ residence;
    3. Current (within 30 days) Gas Bill, showing service address at Westerville Schools’ residence;
    4. Current (within 30 days) Electric Bill, showing service address at Westerville Schools’ residence;
    5. Current (within 30 days) Landline Phone Bill, showing service address at Westerville Schools’ residence;
    6. Current (within 30 days) Cable Bill, showing service address at Westerville Schools’ residence;
    7. Current (within 30 days) Internet Bill, showing service address at Westerville Schools’ residence; or
    8. Current (within 30 days) Government Mailing (i.e., child support, government assistance)
